All personal training programs begin with an initial assessment visit. During the assessment clients will be asked to complete a short questionnaire. The purpose of this questionnaire is to help the trainer and the client establish goals, expectations,

as well as discuss any health issues that need special attention. After the questionnaire has been completed and reviewed, measurements will be taken, such as, body fat %, resting heart rate and body part circumference measurements will be performed. These measurements give a baseline that will be looked at during different points of a clients training cycle to allow better adjustments in training by both the trainer and the client. Once the measurements are over, the fun part begins!
Clients will be taken through a light to moderate initial workout that will encompass the whole body. This allows for identification of strengths and weaknesses a client may possess and further allows the program to be tuned to those needs. While the initial assessment will take approximately an hour and a half, it is only performed at the first meeting. Upon conclusion of the initial assessment, clients training sessions will be made up of 1 hour sessions with a combination of weight training and cardio training, 1,2,3, 4 or 5 times a week based on the program the client chooses. Future assessments will be conducted to assess results and set new goals and targets as deemed necessary between the trainer and client. There is a one time $50 cost for the initial assessment. All additional assessments after the initial will be scheduled and performed at no charge as needed to measure results and progress. Although body weight is checked during the initial assessment, that is not considered to be a measurement that indicates a level of fitness.
***REMEMBER - body weight is never as important as body composition.
By taking a body weight measurement along with body fat%, it will later allow the fat loss and the lean mass gain in the body to be compared.
Training Programs offered by Perfect Form Personal Training:
1 day per week -this program involves training only 1 time a week with a total body training session. While results are not as quickly seen in this program as they are in programs involving more frequent sessions, noticeable improvement can be obtained within a few months .
Cost: $50 per session
2 days per week - this program is a 3 body part per session, twice weekly split, usually consisting of either upper body or lower body and cardio training. The workouts are designed for people who do not have a lot of time, but want to get the best possible performance in the short amount of time they have.
Cost: $45 per session ($360 per month, 8 sessions)
3 days per week - this is the most effective program for increasing general fitness levels with quick, lasting results. The 3 days per week sessions involves a 2 body part per day routine that will develop muscle balance and aerobic conditioning. Visible results are often seen within 3 weeks of beginning this program. Although this is a 3 day per week programs, additional days are available upon request.
Cost: $25 per session ($300 per month, 12 sessions)
*** We also offer specialized training for target specific results. This training is for anyone interested in off-season strength and conditioning programs, such as high school athletes, or anyone with particular development targets such as; power lifting, strength development or body building.
